Job Summary
This role involves delivering medical care and treatment services with a focus on chronic disease management and health promotion for patients who are not best served in a traditional clinic environment. Care is provided in patient residences, as well as through audio and video consultations. This position may concentrate on one of the following areas:
Ongoing Care: Offers continuous care for patients within an assigned caseload and/or territory, emphasizing long-term strategies for disease prevention.
Urgent Care: Provides immediate and/or short-term care for patients, focusing on covering for providers who are unavailable.
Key Responsibilities
Clinical Services
- Conducts diagnosis, treatment, consultation, and follow-up care for patients in collaboration with a licensed physician, addressing critical, prolonged, and long-term healthcare issues.
- Performs comprehensive health assessments, including physical and psychosocial histories, to identify actual and potential health needs based on various factors.
- Identifies, evaluates, and addresses disease prevention, chronic disease maintenance, and appropriate end-of-life care according to patient and family goals.
- Develops and implements proactive, patient-centered care plans with input from other disciplines within the care team.
- Orders diagnostic tests, prescribes medications with appropriate follow-up, and performs certain surgical procedures as permitted by certification.
- Approves medication refills and home health orders, responding to caregiver requests to ensure continuity of care.
- Provides compassionate guidance through chronic disease progression and delivers high-quality palliative care focused on patient and family goals.
- Educates patients and families on goal-oriented, appropriate preventive care, medical issues, and the use of prescribed treatments.
- Builds and maintains relationships with caregivers and care facilities to ensure excellent continuity of care.
- Delivers quality care while adhering to all company productivity metrics.
- Participates in after-hours call schedules.
Administrative Duties
- Communicates effectively with medical staff regarding consultations, collaborations, and referrals.
- Educates patients and families on preventive care and medical issues.
- Assesses the need for specialty and rehabilitative services and makes appropriate referrals.
- Oversees care plans and coordinates case management activities for assigned patients.
- Attends and participates in required staff meetings and other committees as assigned.
- Provides coverage for clinical colleagues in their absence.
- Engages in quality improvement activities as assigned.
- Maintains medical records in accordance with agency protocols and quality metrics.
- Performs other assigned duties.

Experience and Education
Required:
- Master's degree in a primary care program for physician assistants or a related field.
- Licensed physician assistant in the State of Oregon.
- Drug Enforcement Administration (DEA) license.
- Certification by the National Commission on Certification of Physician Assistants (NCCPA).
- Valid driver's license with an acceptable driving record and automobile liability coverage.
- CPR certification at hire or within six months of starting the position.
Preferred:
- A minimum of two years of relevant clinical experience.
Knowledge, Skills, and Abilities
Knowledge:
- Understanding of medical principles and practices, including urgent care and triage.
- Proficient in geriatrics and palliative care.
- Familiarity with general medicine and surgery practices.
- Knowledge of diagnostic and treatment methods for various diseases and injuries.
- Awareness of current literature and information sources in general medicine.
- Understanding of ethical and legal aspects of medical practice.
- Knowledge of public health principles, including epidemiology and health services delivery.
- Familiarity with the population served, including cultural and demographic factors.
- Understanding of clinical quality improvement principles.
- Experience with medical charting systems.
Skills and Abilities:
- Proficient in utilizing electronic systems effectively, including audio and video consultations.
- Ability to perform medical diagnostic and treatment services.
- Capability to establish and maintain effective working relationships with diverse patients and staff.
