Job Summary
We are seeking a skilled Case Manager II to join our rehabilitation care team. In this role, you will work closely with an interdisciplinary team to coordinate patient care from admission to post-discharge. You will be responsible for establishing tentative discharge plans and contingency plans, monitoring patient experience, and facilitating team conferences. You will also be responsible for completing case management addendums and required documentation, maintaining knowledge of regulations and company policies, and reviewing and analyzing case management reports.

Key Responsibilities
The successful candidate will have a strong understanding of rehabilitation care principles and practices. They will be able to communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination. The ideal candidate will also have excellent organizational and time management skills, as well as the ability to work independently and as part of a team.
