As a Physician Assistant, you will be responsible for rounding and managing the clinical care of the population at the assigned SNF, prescribing medications and treatments, and providing high-quality, patient-centered care.

Key Responsibilities
  • Rounding daily to assess patient needs and develop care plans
  • Prescribing medications and treatments as needed
  • Providing comprehensive primary care services to patients residing in SNFs
  • Timely documentation of all patient care/encounters
  • Availability for phone/secure messaging consultations with facility nursing staff
Requirements
  • Current licensure as a Physician Assistant
  • Experience working in skilled nursing facilities or post-acute settings
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Reliable transportation
