Head of Patient Care Operations

Equip Human Solutions is a prominent provider of in-home healthcare services, focused on delivering exceptional care to patients within their residences. Our mission is to improve the health and quality of life of our patients through dedicated and professional support.
Position Summary: We are in search of a skilled and seasoned Director of Patient Care Services. The ideal candidate will be a Registered Nurse (RN) with significant experience in home health care. This pivotal role encompasses the management of clinical operations, ensuring the utmost standards of patient care, and adhering to all regulatory guidelines.
Key Responsibilities:
- Direct and supervise the patient care services team, which includes nurses, therapists, and various healthcare professionals.
- Formulate and execute clinical policies and procedures to guarantee superior patient care.
- Assess and review patient care services to ensure adherence to state and federal regulations.
- Oversee the creation and execution of individualized patient care plans.
- Facilitate regular staff meetings and provide continuous training and professional development opportunities.
- Collaborate with healthcare providers, patients, and families to ensure comprehensive patient care coordination.
- Manage the performance improvement initiatives to enhance patient outcomes.
- Ensure precise and timely documentation of patient care services.
- Act as a resource and mentor for clinical staff, offering guidance and support as necessary.
- Valid and unrestricted Registered Nurse (RN) license in the state of CA.
- At least 5 years of experience in home health care.
- Demonstrated leadership and management experience in a clinical environment.
- In-depth knowledge of home health care regulations and standards.
- Exceptional communication, organizational, and interpersonal skills.
- Ability to thrive in a fast-paced and dynamic setting.
- Proficient in electronic health record (EHR) systems and Microsoft Office Suite.
