Job Summary

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct adult/geriatric assessments, medical history reviews, and provide health education to patients in their homes.

You will work collaboratively with physicians, case managers, social workers, and other healthcare professionals to provide comprehensive care and support to our patients.

Responsibilities

As a Nurse Practitioner with Matrix Medical Network, your responsibilities will include:

  • Conducting adult/geriatric assessments, medical history reviews, and providing health education to patients in their homes
  • Working collaboratively with physicians, case managers, social workers, and other healthcare professionals to provide comprehensive care and support to our patients
  • Providing services in a variety of venues, including home visits, skilled facility visits, and telehealth visits
Requirements

To be considered for this role, you must have:

  • A Master's Degree in Nursing or a related field
  • Current RN and NP licensure in the state of practice, including prescription authority
  • Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
  • A minimum of 1 year of experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health record software
  • Excellent verbal and written communication skills
