Key Responsibilities- Perform home visits and coordinate client care to ensure the best possible outcomes for our patients.
- Oversee the frequency of visits for each episode and consult with physicians as needed to ensure accurate and timely care.
- Collect and document accurate OASIS data, maintain up-to-date medication profiles, and report lab values to physicians in a timely manner.
- Attend weekly case conferences and monthly case manager meetings to stay informed and collaborate with colleagues.
- Current licensure as a Registered Nurse (RN) in the state of employment.
- A minimum of one year of clinical experience preferred.
- Demonstrate knowledge and skill in current nursing practice.
- Valid state driver's license and automobile liability insurance.
