Your Responsibilities:

  • Delivering personal care services such as bathing, skin care, oral hygiene, and changing linens
  • Assisting patients with dressing and undressing
  • Planning and preparing healthy meals, including grocery shopping
  • Helping patients with feeding
  • Monitoring and caring for patients' skin to prevent any tissue damage
  • Communicating any changes in the patient's condition to the Case Manager.

Required Qualifications:

  • Current certification as a CNA is mandatory.
  • Experience in hospice care is preferred.
  • Demonstrates a compassionate demeanor towards patients and their families.
  • Ability to comprehend and follow written instructions and document care provided.
  • Must possess a valid CPR certification.
  • Capable of lifting up to 50 pounds.
  • Must hold a valid driver's license and have access to a reliable, insured vehicle.
