Job Summary

Alternate Solutions Health Network LLC is seeking a skilled Occupational Therapist to join our team in providing high-quality home health care services. As a key member of our interdisciplinary team, you will play a vital role in helping patients achieve their maximum potential and independence.

Key Responsibilities
  • Develop and implement individualized occupational therapy plans to meet the unique needs of each patient.
  • Conduct thorough assessments and evaluations to identify patient goals and objectives.
  • Provide skilled occupational therapy services, including activities and home exercise programs, to improve range of motion, muscle strength, and coordination.
  • Collaborate with patients, families, and healthcare professionals to ensure comprehensive care and support.
  • Document patient progress and outcomes in accordance with agency protocols and Medicare/Federal guidelines.
Requirements
  • Current licensure as an Occupational Therapist in the state of employment.
  • Minimum one year of experience as an Occupational Therapist in an acute care setting, with home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Ability to meet the physical demands of the job.
