STGi is currently in search of a Licensed Practical Nurse / Licensed Vocational Nurse to deliver essential services at our Community Based Outpatient Clinic. The primary responsibility of the Licensed Practical Nurse / Licensed Vocational Nurse is to offer a diverse range of clinical and administrative nursing care under the supervision of the CBOC Clinic Manager, PACT Provider, and STGi Program Manager.

KEY RESPONSIBILITIES:

  • Exhibit fundamental knowledge of pharmacology and medication administration techniques in accordance with facility protocols and state regulations, as directed by the PACT Provider.
  • Collaborate with the PACT team for patient check-in and check-out processes.
  • Engage in all staff meetings and PACT huddles.
  • Monitor patients and report any adverse reactions to medications, treatments, or changes in physical/mental condition.
  • Administer cardiopulmonary resuscitation (CPR) and assist during respiratory and cardiac emergencies.
  • Execute a variety of nursing procedures, including dressing changes, suture removals, oxygen administration, wound irrigation, catheterization, injection administration, and venipunctures.
  • Prepare patients for laboratory tests and examinations by providing necessary information and instructions, while assisting the Primary Care Provider and RN Care Manager as needed.
  • Set up and operate therapeutic and emergency equipment, including monitors, EKGs, oxygen systems, automatic electronic defibrillators (AEDs), and portable suction devices.
  • Uphold universal precautions and infection control practices.
  • Provide assistance to support staff as required.
  • Accurately document all relevant patient information in their medical records and VISTA to ensure quality care delivery and continuity of care.
  • Maintain the VISTA clinic reminder tracking system for each enrolled patient.
  • Complete all necessary documentation as outlined by the site-specific VAMC for compliance with the Directors Performance Measures.
  • Assist in scheduling initial and follow-up appointments based on VA access standards and the patient's medical necessity.
  • Ensure patients are informed about their care plans, including lab and diagnostic test results.

Deliver health education materials and resources to patients and their families for informational purposes.

  • Participate in the ongoing Performance Improvement Program between HNFS, its Subcontractor(s), and the VAMC.
  • Aid in ensuring that all required reports are completed accurately and thoroughly.
  • Support the CBOC Clinic Manager (either PA/NP or RN Care Manager) in maintaining clinic compliance with all federal, state, local, JCAHO, OSHA, Veteran Affairs, STG VA Program, and Subcontractor safety and operational regulations.
  • Assist the RN Care Manager in ensuring that all patient information regarding access/wait time documentation is complete and accurate.
  • Maintain confidentiality of all information and uphold patients' privacy, rights, and safety.
  • Apply basic customer service techniques, cultural diversity, and age-specific considerations.
  • Document patient reminders, clinical interactions, and close encounters, addressing inquiries, and performing other duties as necessary to ensure a productive visit.
  • Conduct follow-up visits.
  • Provide general clerical and administrative support for the unit.
  • Perform additional work-related duties as assigned.

QUALIFICATIONS:

  • Minimum of 1 year of current clinical experience in a similar environment, with outpatient clinical and/or hospital experience preferred (Medical-Surgical, Intensive Care Unit, Emergency Room, etc.).
  • Knowledge of basic and complex methods for coordinating and scheduling clinical care.
  • Comprehensive understanding of anatomical variations.
  • Proficiency in applying practical nursing principles, procedures, and techniques.
  • Ability to assess patient conditions, recognize adverse signs and symptoms, and respond promptly in emergencies.
  • Capability to follow orders and work under close supervision while demonstrating initiative and independence.
  • Ability to provide individualized and group counseling, guidance, and health instruction focused on disease prevention and health maintenance as directed by the provider.
  • Competence in providing educational materials to patients and families regarding proper home health care activities.
  • Familiarity with pharmacological agents used in patient treatment, including their effects and considerations.
  • Understanding of customer service principles related to age-specific populations.
  • Strong verbal and written communication skills along with proper telephone etiquette.
  • Knowledge of specialized terminology associated with the work in the facility and in-depth knowledge of terminology used in Primary Care Practice.
  • Excellent customer service skills.
  • Proficiency in Microsoft Office Software, Electronic Medical Records, and general computer navigation.
  • Exceptional diplomatic communication skills and experience in managing challenging customer situations.
  • Detail-oriented with the ability to multitask.

EXPERIENCE REQUIRED:

  • LPN from an accredited nursing school.
  • Current and unrestricted nursing license in the state of practice.
  • Maintain current Cardiopulmonary Resuscitation (CPR) / Basic Life Support (BLS) certification from AHA or MTN.

WORKING CONDITIONS:

  • Moderate physical activity, requiring handling of average-weight objects up to 25 pounds or standing and/or walking for more than four (4) hours per day.
  • Work is typically performed in a standard interior/office environment.
  • Required to respond to any medical emergency within the clinic, necessitating a fast walk or jog to provide assistance.
