Speech Language Pathologist - Home Health

Adventist HealthCare is seeking a skilled Speech Language Pathologist to join our Home Health team. As a Speech Language Pathologist, you will play a vital role in providing high-quality care to patients in their homes, helping them to recover from illness or injury and regain their independence.

Responsibilities:
  • Conduct initial examinations and evaluations to establish a diagnosis, prognosis, and plan of care for patients with speech and language disorders.
  • Develop and implement individualized therapy plans to address patients' specific needs and goals.
  • Provide speech and language therapy services to patients in their homes, using a variety of techniques and strategies to promote communication and swallowing skills.
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care and coordination of services.
  • Document patient progress and outcomes, and communicate with patients, families, and healthcare teams to ensure seamless care.
Qualifications:
  • Master's or Doctoral degree in Speech-Language Pathology from an accredited program.
  • Current CCC-SLP certification.
  • 1 year of experience in home health or a related setting.
  • Valid driver's license and reliable transportation.
  • Active American Heart Association Basic Life Support (BLS) certification.
