Job Summary

We are seeking an experienced Hospice Registered Nurse (RN) to join our team. As a Hospice Nurse, you will be responsible for delivering high-quality patient care and collaborating with our interdisciplinary team to ensure the best possible outcomes.

Key Responsibilities
  • Apply a working knowledge of nursing theories and concepts to provide patient-centered care.
  • Coordinate physical care of the patient with family, patient, physician, and Hospice team.
  • Communicate effectively with patients, families, and healthcare professionals.
  • Participate in IDT meetings and quality review as scheduled by the agency.
Requirements
  • Current RN licensure in the state of practice.
  • Successful completion of a nursing education from an approved school of nursing.
  • At least two years of clinical nursing experience and one year of hospice nursing experience preferred.
  • Knowledge of home health and hospice care programs and the nurse's role as a member of the patient care team.
  • Working knowledge of nursing principles and practice according to nursing practice act, agency policies, and procedures.
  • 12 hours of continuing education annually.
  • CPR certification.
  • Ability to accept travel assignments and weekend/evening and holiday call duties.
