We are seeking a highly skilled Clinical Care Coordinator to join our team at Regional Cancer Care Associates. The successful candidate will be responsible for coordinating the development of evidenced-based practice protocols and standards of care that include interventions and treatments to achieve desired outcomes, including clinical, financial, and patient/family satisfaction.

About the Role

The Clinical Care Coordinator will have a Master's degree in nursing and be certified as an APN in New Jersey. They will have experience in relevant patient care settings and maintain national certification in their area of specialty. The ideal candidate will be able to analyze patterns of variance in practice making recommendations for evidence-based changes to improve patient outcomes utilizing a multidisciplinary approach.

Key Responsibilities
  • Analyzes patterns of variance in practice making recommendations for evidence-based changes to improve patient outcomes utilizing a multidisciplinary approach.
  • Clinically coordinates/participates in formal or informal educational programs which include mentoring.
  • Serves as a clinical resource person to nursing staff, patients/families, and other multidisciplinary team members across the continuum to evaluate complex patients and identify appropriate desired outcomes and interventions.
