We are seeking a dedicated and compassionate Home Care Nurse to join our team at Aveanna Healthcare. As a Licensed Practical Nurse (LPN), you will provide high-quality, patient-centered care to our adult and pediatric patients in the comfort of their own homes.
Responsibilities- Implement and oversee the delivery of care according to the physician's orders and the patient's individualized plan of care.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing effective interventions.
- Administer medications, treatments, and therapies as prescribed by physicians, ensuring accuracy and adherence to safety protocols.
- Document patient care activities, progress, and concerns in a timely and accurate manner, maintaining confidentiality and adhering to HIPAA regulations.
- Provide education and training to patients and family members on disease management, self-care techniques, and prevention strategies.
- Collaborate with healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Active, unencumbered LPN license in the state of practice.
- Current CPR certification.
- TB skin test (current within the last 12 months).
- At least six months of recent experience as a Licensed Practical Nurse in a clinical care setting (home health experience a plus).
