Compassionate Care Provider

We are seeking a compassionate and skilled Nurse Practitioner to join our team in providing exceptional care to patients with life-limiting illnesses. As a member of our hospice and palliative care team, you will work collaboratively with a multidisciplinary team to deliver high-quality, patient-centered care.
Responsibilities- Provide comprehensive and compassionate care to patients and families in their homes, hospitals, and other settings.
- Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Advocate for patients and families, addressing their concerns and needs in a supportive and empathetic manner.
- Stay up-to-date with best practices in palliative and hospice care, ensuring high-quality care delivery.
- Promote a culture of respect, empathy, and compassion within the team and across all interactions.
- A heart for serving patients and families with compassion and care.
- Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
- Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred.
- Reliable transportation and ability to sit, stand, bend, move intermittently, and lift at least 25 lbs.
