Job Summary

Key Responsibilities
  • Direct patient care services, ensuring seamless integration with the interdisciplinary plan of care.
  • Review patient admissions, assessments, and discharges to ensure eligibility and appropriateness criteria are met.
  • Collaborate with physicians to develop and implement palliative care measures for pain and symptom management.
  • Ensure accurate and timely data documentation to inform recertification decisions.
  • Monitor team services to ensure they meet patient needs and expectations.
  • Manage team schedules and coordinate services 24/7 to meet patient needs.
  • Facilitate team meetings to ensure interdisciplinary input into patient care planning.
  • Participate in on-call rotation to ensure continuity of care.
Requirements
  • Active RN license
  • At least 2 years of RN experience
  • 1+ years of management experience
  • Hospice, assisted living, or long term care experience (preferred)
