BAYADA Home Health Care is seeking a skilled Pediatric Nurse to join our Hospital at Home team. As a Pediatric Nurse, you will provide high-quality, patient-centered care to infants and young children in the comfort of their own homes.
Key Responsibilities- Deliver skilled nursing care to pediatric patients in their homes, working collaboratively with the hospital care team
- Conduct thorough assessments, develop and implement individualized care plans, and provide ongoing education to patients and families
- Collaborate with the multidisciplinary team to ensure seamless transitions of care and optimal patient outcomes
- Document patient progress, identify areas for improvement, and participate in quality improvement initiatives
- Current licensure as a Licensed Practical Nurse (LPN)
- At least one year of verifiable work experience as an LPN, preferably in NICU, PICU, ICU, or home care settings
- Demonstrated competence in infusion and tracheostomy/ventilator care
