Luzerne County, PA is seeking a highly skilled Care Management Specialist to join our team. As a Care Management Specialist, you will be responsible for providing a wide range of care management activities to a caseload of elderly consumers and their caregivers.
Key Responsibilities:- Conduct home visits to initiate care plans for consumers and provide education on available community resources.
- Conduct reassessment activities, including adjusting care plans and cost share program calculations, to ensure consumers receive the necessary support.
- Respond to emergency situations, monitor services, and provide emotional support to consumers.
- Enter data into the state-wide SAMS system and maintain accurate records.
- Assist consumers in navigating the Aging service system and advocate for their needs.
- Arrange for financial management and housing/environmental support for consumers.
- Participate in consumer Appeals Hearings and serve as a back-up to telephone Intake and Referral care managers.
- Knowledge of social casework principles and methods.
- Knowledge of community support services for older adults.
- Ability to establish and maintain effective working relationships with clients and staff.
- Ability to plan and organize work, prepare records, and maintain a caseload.
