Position Overview:

The Certified Healthcare Assistant plays a crucial role in collaborating with the medical team to ensure high-quality and comprehensive patient care.

Key Responsibilities:

  • Effectively communicates health-related information to patients, support personnel, and team members.
  • Handles patient inquiries via phone, documenting and relaying questions in Electronic Health Records (EHR) to the provider as necessary.
  • Conveys relevant information from providers to patients as documented in EHR.
  • Supports providers in educating patients about their health.
  • Facilitates and processes medication refill requests in coordination with the provider.
  • Monitors and manages voice mail, EHR messages, and emails promptly.
  • Assists the care team in assessing patients and identifying health conditions.
  • Coordinates patient care, referrals, and transitions in collaboration with other members of the Family Health Centers, Inc. care team.

Patient Care Duties:

  • Delivers nursing care to patients in accordance with established standards and practices, adhering to the scope of the LPN license or MA-C certification.
  • Records vital signs and patient medical history accurately.
  • Administers medications as prescribed, including legend drugs, vaccines, and controlled substances, following a healthcare practitioner's written order.
  • Conducts diagnostic tests limited to electrocardiography and respiratory testing, as well as tests permitted under the federal clinical laboratory improvement amendments (CLIA) program.
  • Prepares for and assists with medical procedures according to clinic protocols and policies.
  • Implements standing orders for patients as authorized by the Medical Director.
  • Regularly checks refrigerator temperatures to ensure proper medication storage.
  • Triage patients according to the level of employee licensure, assessing needs and providing appropriate nursing care.

Professional & Technical Skills:

The role requires a foundational level of written and verbal communication skills, computational abilities, and mathematical knowledge typically acquired through high school education. Specific knowledge and training as a Licensed Practical Nurse, usually obtained through formal education, are essential. Additionally, expertise in Medical Assistance is necessary, typically gained through completion of a Medical Assistant certification program lasting up to one year.
