Job Overview
The Wellness Nurse is responsible for the overall well-being of our residents at Sunrise Villa Tuckahoe. This role ensures that our residents receive the highest quality resident services, promoting their health and wellness.
Responsibilities
* Identifies ongoing needs and services of residents, ensuring proper follow-through and documentation.* Assists the Resident Care Director (RCD) in conducting resident evaluations and gathering information.* Conducts monthly wellness visits for all residents.* Monitors and follows up with residents who have experienced any fall, accident, or incident; documents and reports as required.* Maintains communications with residents' family members and responsible parties to ensure medical needs are met.* Contacts residents' attending physicians when necessary and/or upon family request.* Ensures weights and vital signs are obtained monthly for each resident.* Assists in maintaining wellness files according to Sunrise policies and federal regulations.* Maintains medical supplies and emergency kits for the community.* Provides clinical support and assistance to community team members as needed.* Understands and follows infection control practices and promotes infection control standards within the community.
Requirements
* Graduate of an approved college/school of nursing.* Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).* Minimum of one year of experience in home health, assisted living, hospital, or long-term care environment.* Demonstrates knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, as well as federal, state, and local regulations.* Ability to handle multiple priorities and possess written and verbal skills for effective communication.
