Career Opportunities

We are currently seeking an experienced Registered Nurse (RN) to join our team as an Advanced Care Planning Coordinator PRN. This role will play a crucial part in providing consultation and education to patients and families regarding palliative care services.

About the Role

This is a per-diem position, allowing for flexibility and autonomy in scheduling. As an Advanced Care Planning Coordinator, you will work closely with our interdisciplinary team to ensure seamless coordination of patient care. Your primary responsibilities will include:

  • Maintaining a knowledge base on current trends influencing healthcare practice in palliative care
  • Participating in performance improvement activities to enhance patient outcomes
  • Serving as a clinical liaison for palliative care services
  • Demonstrating expertise in various financial classes and regulations to facilitate patient transitions
  • Playing an active role in discharge planning

Requirements

To be considered for this role, you must possess:

  • A graduate degree in nursing from an accredited institution, preferably a Bachelor of Science in Nursing (BSN)
  • A valid license to practice as a Registered Nurse in the Commonwealth of Virginia or a compact state
  • BLS certification
  • At least two years of experience as a RN, preferably in a specialty area such as case management or oncology/hospice nursing
