Geriatric Nurse Specialist

Vaya Health is seeking a highly skilled and experienced Geriatric Nurse Specialist to join our team. As a key member of our Geriatric and Adult Mental Health Specialty Team, you will play a critical role in providing training, consultation, and technical assistance to community organizations and staff of nursing homes, adult care homes, and family care homes serving adults with mental illness.
Key Responsibilities- Develop and teach evidence-informed presentations about mental illnesses and how to manage difficult behaviors to community organizations and staff.
- Provide education and linkage to services for caregivers of older adults who are providing care in the community.
- Provide training to staff and volunteers on topics relevant to older adult mental health, including recognizing symptoms of mental illness, behavioral intervention, and communication issues.
- Assist staff in assessing behaviors and provide case consultation regarding behaviors that may result in the need for more intensive services.
- Model for staff and provide technical assistance with implementation of intervention plans.
- Assist staff or caregivers with linkage to community providers/resources serving the geriatric population for the purpose of promoting aging in place and improvement of health.
- Work collaboratively with other team members on clinical issues, including diagnostic criteria and evidence-based treatment options for individuals with serious and persistent mental illnesses.
- Participate in community workgroups to enhance the community's ability to provide services/care for older adults.
- Document required elements on Division of MHDDSUS reports, in Vaya electronic health record, and other internal processes.
- Associate degree in Nursing required and at least 1 year experience working with older adults with mental health and/or substance use disorders.
- Active, unrestricted license to practice as a Registered Nurse in North Carolina by the NC Board of Nursing.
- Sedentary work with lifting requirements up to 10 pounds, sitting for extended periods of time.
- Mental concentration is required in all aspects of work.
- Ability to drive and sit for extended periods of time (including in rural areas).
