**Job Summary:** We are seeking a skilled and compassionate Home Health Registered Nurse to join our team at Protouch Staffing. As a Home Health Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.
Key Responsibilities:
- Assess patients' physical, psychosocial, and environmental needs to identify areas for improvement and develop effective care plans.
- Implement and evaluate care plans, ensuring continuity and quality of care.
- Provide education and support to patients and their families on self-care practices, medication management, and home safety.
- Supervise and provide clinical direction to Home Health Aides and other support staff.
- Collaborate with healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Monitor and report patient progress, identifying areas for improvement and making recommendations for adjustments to care plans.
- Participate in interdisciplinary team meetings and contribute to the development of comprehensive care plans.
- Perform other related duties as assigned or requested.
Requirements:
- Current state license as a Registered Nurse.
- 1-2 years of experience as a Registered Nurse in a clinical care setting.
- 2+ years of experience in home health care.
- Current CPR certification.
- Reliable transportation and insurance.
- Ability to work flexible schedules and travel locally.
- Ability to maintain confidentiality and work independently as an effective team member.
Preferred Qualifications:
- Bachelor's of Science in Nursing.
- CHPN certification.
