Responsibilities- Develop and implement individualized care plans, working closely with guests, families, and healthcare professionals to ensure comprehensive care.
- Supervise and mentor Certified Nursing Assistants (CNAs) and other team members, promoting a culture of excellence and teamwork.
- Administer medications and treatments, ensuring accurate and safe delivery of care.
- Conduct thorough assessments, identifying and addressing the unique needs of each guest.
- Collaborate with the interdisciplinary team to develop and implement care plans, ensuring seamless transitions and optimal outcomes.
- 1-3 years of experience in a long-term care setting, with a strong background in rehabilitation and skilled care.
- Current LPN licensure in the state, with CPR certification.
