About the Role

Catholic Health Initiatives is seeking a skilled Social Worker to join our Home Health and Hospice team. As a Social Worker, you will play a vital role in providing compassionate care to patients in their homes, working closely with our interdisciplinary team to develop and implement personalized care plans.

Key Responsibilities

  • Assess the psychosocial status of clients and communicate findings to the registered nurse
  • Carry out social evaluations and plan interventions based on evaluation findings
  • Maintain clinical records on all clients referred to social work
  • Provide information and referral services to clients and families/caregivers regarding environmental needs
  • Market and promote agency programs and events in the local community

Requirements

  • Bachelor's degree in Social Work
  • Licensed as a Social Worker in Minnesota and North Dakota
  • One year experience in healthcare
  • Excellent verbal, written communication skills and organizational skills
