Purpose and Scope:

As a key member of the Fresenius Medical Care North America team, the Social Work Services Manager plays a vital role in supporting the organization's mission, vision, and core values. This position is responsible for providing leadership and guidance to facility master social workers in an assigned area, ensuring the delivery of excellent behavioral health care to patients.

Key Responsibilities:

Serves as a subject matter expert for psychosocial matters, collaborating with facility MSWs and the Area Quality Team to improve patient clinical and quality health outcomes.

Identifies clinical quality improvement opportunities in the assigned area, aligning with CMS Conditions for Coverage and FMS quality program requirements.

Manages the tactical execution of Quality and Social Work initiatives, interventions, and standardized supportive counseling and education materials with facility MSWs within the assigned area.

Provides general direction, social work guidance, and feedback to facility MSWs, ensuring they meet professional standards and FMCNA guidelines.

Leads, coaches, and develops direct reports, maintaining an engaged and productive workforce, and partnering with Human Resources on employee matters.

Manages the staffing of Social Work Services, ensuring master social workers' coverage during times of leave, vacations, census increases, and vacancies.

Collaborates with management staff to achieve effective inter-disciplinary, intra-disciplinary, and clinic relationships.

Monitors and audits Social Work Progress Notes, Comprehensive Interdisciplinary Assessments, Plans of Care, and other documentation to meet CMS Conditions for Coverage and FMS policies.

Requirements:

Masters in Social Work required

State Specific Licensure required

2-3 years dialysis experience required

3+ years' supervisory or project/program management experience preferred

Licensure to provide therapeutic counseling and supervision, preferred

Proficient with Microsoft applications, including Outlook, Word, Excel, PowerPoint, and other web-based applications.

Demonstrated leadership competencies and skills, including ability to teach, strong organizational skills, excellent communication, customer service, relationship development, results orientation, team building, and decision making.
