We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Oak Street Health Inc. As a key member of our healthcare team, you will provide exceptional primary care to our patients, focusing on their overall health and well-being.
Key Responsibilities- Conduct thorough office visits for routine and acute issues, administering Annual wellness visits and health risk assessments.
- Care coordination with other providers, specialists, testing facilities, and agencies to ensure comprehensive patient care.
- Participate in population health leadership, collaborating with the Care Team to promote health and prevent disease.
- Assist with phone triage and outreach, educating patients on their health conditions, care plans, and treatments.
- Participate in Oak Street Health promotional activities and conduct home visits as needed.
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
- National certification as a Family, Adult or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
- Active, non-probationary state Nurse Practitioner license and Active DEA license.
- US Work Authorization and a passion for teamwork and collaboration.
- Experience in primary care: internal medicine, geriatrics, or family medicine.
- Understanding of HCC documentation, ICD-10 Coding, and Health Risk Assessments.
- Passion for population health and a desire to be part of an innovative model.
