Avera seeks a Registered Nurse (RN) Case Manager to deliver exceptional nursing care in its patient-centered settings. As a key member of the RN Nursing team, you will support patient care navigation and manage care for patients in the clinic setting.

Key responsibilities include developing and monitoring care management processes, supporting primary clinical teams, and identifying high-acuity patient populations. You will work closely with the AMG Director of Patient Care Coordination/Medical Home and others to develop quality measures within the medical home model.

With a strong background in nursing, including at least 3 years of clinic nursing experience, you will be responsible for demonstrating performance consistent with professional standards of practice and the Nurse Practice Act. A Bachelor's degree in nursing is preferred, and bilingual Spanish proficiency is desirable.

This role offers a competitive salary range of $83,000 - $110,000 annually, depending on experience, as well as benefits such as health insurance, paid time off, and retirement plans.
