Clinical Pharmacist

Overview: Healthy Community Health Center (HCHC) seeks a highly skilled and dedicated Clinical Pharmacist to join our team. As a member of our healthcare team, you will play a crucial role in providing patient-centered care, ensuring the safe and effective use of medications, and promoting healthy outcomes.

Job Summary: The Clinical Pharmacist will be responsible for dispensing prescribed medications, providing clinical pharmaceutical care, and collaborating with healthcare providers to ensure comprehensive patient care.

Key Responsibilities:

• Dispense prescribed medications while adhering to state and federal regulations.
• Provide clinical pharmaceutical care to patients, including medication therapy management and education.
• Collaborate with healthcare providers to ensure coordinated patient care.
• Participate in resolving medication-related issues and optimizing patient outcomes.
• Develop and implement patient-specific treatment plans in collaboration with healthcare teams.
• Maintain accurate and up-to-date patient records.
• Comply with regulatory requirements and maintain licensure as a Registered Pharmacist.

Requirements:

• Meets the requirements for licensure as a Registered Pharmacist in the Commonwealth of Virginia.
• Completes pharmacy-based immunization certificate or equivalent within 6 months of employment.
• Possesses excellent communication and interpersonal skills.
• Maintains confidentiality and discretion when handling sensitive patient information.
• Ability to work effectively in a fast-paced environment and adapt to changing situations.

Salary: $120,000 - $150,000 per annum, depending on experience.
