Job Summary

The Social Worker is a key member of the Luminary Hospice clinical team, responsible for assessing the psychosocial status of patients and their families/caregivers in relation to their terminal illness and environment. This role requires strong communication and interpersonal skills to effectively collaborate with the interdisciplinary team and provide high-quality patient care.

Key Responsibilities
  • Assess the psychosocial status of patients and their families/caregivers, identifying areas of need and developing interventions to address these concerns.
  • Provide counseling and support to patients and their families/caregivers, addressing stress, coping difficulties, and other psychosocial issues.
  • Collaborate with the interdisciplinary team to develop and implement individualized plans of care, ensuring that the patient's and family's needs are met.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Participate in quality assessment and performance improvement activities to enhance patient care and outcomes.
Requirements and Qualifications
  • Bachelor's degree in social work or a related field, with a Master's degree in social work (MSW) preferred.
  • Active licensed social worker (LSW) or licensed clinical social worker (LCSW) certification.
  • At least 1-3 years of experience in healthcare management, hospice, or palliative care, with training in end-of-life care preferred.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work effectively in a fast-paced, dynamic environment.
Work Environment

The work environment for this role is typically quiet, with occasional opportunities to work in patient homes or other community settings. Reliable transportation and an active driver's license are required, as this role involves frequent travel to patient homes.
