We are seeking a highly skilled Transition Coordinator to join our team at Oakland Schools. The successful candidate will be responsible for providing vocational evaluation, transition planning, and employment skills development for students with special needs.
Key Responsibilities- Provide vocational evaluation and written reports for secondary students with special needs.
- Develop and implement effective transition plans in collaboration with students, parents, and staff.
- Provide vocational counseling and assist students in acquiring basic employment skills.
- Organize and implement the school's Work-Based Learning Program, ensuring student safety and success.
- Collaborate with community transition agencies and special services staff to provide comprehensive support.
- Train and direct paraprofessionals and teachers to support transition programming.
- Participate in IEP meetings and contribute to the development of individualized education plans.
- Maintain accurate records and reports, including Student Information Binders and transition plans.
- Develop and maintain relationships with families, adult service providers, and community partners to ensure successful student transitions.
- Bachelor's degree in special education from an accredited institution.
- Minimum of 3 years of satisfactory teaching experience in special or vocational education.
- Preferred experience in transition planning, youth employment, and vocational assessment of students with special needs.
