We are seeking a skilled and compassionate Physical Therapist to join our team at Interim HealthCare of Oakland County. As a Home Health Physical Therapist, you will play a vital role in providing personalized care to patients in the comfort of their own homes.
Key Responsibilities- Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury.
- Work collaboratively with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to develop and implement patient care plans.
- Conduct thorough assessments, document progress, and modify care plans as needed to ensure optimal patient outcomes.
- Assist patients with exercises and hands-on therapy to manage pain and increase mobility.
- Educate patients and their families on the plan of care, established goals, exercises, and self-care techniques.
- Coach family members on safely assisting with exercises and promoting patient independence.
- Graduate of an accredited Physical Therapist Program and active PT license in Michigan.
- Minimum of 2 years of physical therapy experience, preferably in home healthcare.
- CPR certification.
- Knowledge of state and federal home health regulations.
- Good clinical judgment, strong interpersonal skills, dexterity, and compassion.
