ChenMed is a physician-led, mission-driven organization that is changing the face of healthcare for seniors. We are seeking a highly skilled and compassionate Primary Care Physician to join our team.

Key Responsibilities:

  • Provide high-quality, patient-centered care to seniors with acute and chronic illnesses
  • Develop and implement individualized care plans to improve patient outcomes
  • Collaborate with other healthcare professionals to ensure seamless care coordination
  • Lead and manage a team of care promoters, care facilitators, and care coordinators
  • Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements:

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty
  • Current, active MD licensure in the state of employment
  • Minimum 1-year clinical experience in geriatric, adult, or family practice setting
  • Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty
  • Basic Life Support (BLS) certification
  • Current DEA number for schedule II-V controlled substances
