About the Role

As a Full-Time Case Manager within our Renaissance Community program, you will play a vital role in empowering individuals with behavioral health needs. This position combines community-based support with a focus on individual growth and recovery.

Program Overview

The Renaissance Community is a supportive living program serving individuals in Montgomery County. We offer 24-hour supervision in a community setting, emphasizing behavioral health services. Participants reside in either communal homes or apartments based on their needs for structure and support. The program provides opportunities for engagement in activities, groups, day treatment programs, and medication monitoring to help individuals achieve their goals of independent living, learning, working, and social integration.

Your Responsibilities
  • Collaborate with a multidisciplinary team to develop and implement individualized recovery plans for program participants.
  • Connect participants with community resources and support networks to enhance their well-being.
  • Provide direct client care, including community-based services focused on education, resource connection, social interaction, and goal achievement.
  • Complete all required assessments and documentation in accordance with RHD, funder, and licensing/accreditation standards.
Qualifications
  • Bachelor's Degree in Human Services or a related field.
  • Intermediate computer skills.
  • One year of Case Management experience is preferred.
  • Two years of general work experience providing support to individuals in behavioral health, addiction recovery, or intellectual and developmental disabilities (IDD) services is required.
  • Valid driver's license.
