Position Overview

The VNA of the Treasure Coast is dedicated to delivering exceptional and innovative healthcare services, establishing a benchmark for patients and caregivers requiring private care, home health, hospice, and community health support. Our commitment is to ensure the highest quality of patient care.

We are currently seeking an Advanced Registered Nurse Practitioner to deliver comprehensive medical services to patients utilizing the VNA Mobile Unit.

Key Responsibilities
  • Conducts thorough physical assessments and preventive health interventions following established protocols in collaboration with the Medical Director.
  • Educates patients and their families to achieve health objectives. Provides guidance to clinic personnel and offers detailed instructions regarding disease management, medications, and follow-up care.
  • Documents clinical findings and develops treatment plans based on patient evaluations. Initiates and maintains patient records in compliance with Agency, State, and Federal regulations.
  • Refers patients to appropriate healthcare providers and community resources as necessary.
  • Submits patient documentation for regular review and assessment by the Medical Director.
  • Prescribes or recommends medications and treatment modalities in accordance with established protocols. Monitors patient responses to prescribed therapies.
  • Facilitates referrals to physicians for specialized consultations or additional health resources.
  • Engages in multidisciplinary meetings, collaborating with team members to devise innovative solutions for patient care challenges. Assists in the onboarding of new nursing staff and participates in community health initiatives.
  • Supervises mobile clinic personnel to ensure the effective and timely delivery of services in line with program standards. Demonstrates adaptability in expectations and is prepared to adjust strategies as required.
Qualifications
  • Active ARNP license in the state of Florida
  • Must possess a Family Nurse Practitioner certification
  • At least 1 year of experience as a Registered Nurse
