Overview

We're seeking a Registered Nurse (RN) or Physical Therapist (PT) to join our team as a Care Transition Coordinator. In this role, you'll play a critical part in navigating patients through post-acute care, ensuring a seamless transition from acute care to home-based services. Your expertise will be instrumental in developing patient-centered transition plans that promote quality and efficiency in the delivery of post-acute care.

Responsibilities
  • Assist patients in navigating post-acute care options and services
  • Assess, plan, implement, coordinate, monitor, and evaluate transition plans to ensure patient safety and quality care
  • Integrate evidence-based clinical guidelines and preventative measures into transition plans
  • Promote adherence to post-acute plans and ensure ordered services are completed
  • Represent Enhabit in transitional care activities and strategic relationships with health systems, hospitals, inpatient facilities, and physician groups
  • Monitor execution of transitional care services through ongoing quality assurance visits with referral sources
  • Meet and/or exceed referral and admission goals
Qualifications

We're looking for a goal-driven, sales-motivated RN or PT with previous home health or hospice experience. You'll need to be a graduate of an approved school of nursing or therapy and be licensed in the state of employment. A minimum of 2-3 years of field experience is required, along with a strong understanding of customer and market dynamics, transitional care best practices, and regulatory guidelines governing home health and hospice operations.
