At Day City Hospice, we take a patient-centered approach to end-of-life care. Our team of dedicated professionals works together to provide comprehensive support and resources to patients and their families. With our local management team and clinicians, we are able to respond quickly and effectively to the needs of our patients.

Schedule

We offer flexible scheduling options, including 3–12 hour weekday shifts with occasional weekends. Our goal is to ensure that our staff has the time and energy to provide exceptional care to each of our patients.

Job Description

As an End of Life Care Specialist, you will be responsible for performing pre-admission evaluations to determine the appropriateness of services. You will educate and inform patients and families about our services, obtain informed consent, and provide nursing assessments and interventions to optimize patient comfort and quality of life.

Qualifications

  • Nursing Degree – A graduate from an accredited registered nursing program is required.
  • State Licensure – Must be licensed as a registered nurse in the state of practice.
  • CPR Certification – Current certification is necessary.
  • Hospice Experience – One year of hospice experience is preferred.
