As a Wound Care Specialist, you will be responsible for collaborating with our interdisciplinary team to review potential wound care patients and assess their eligibility for admission. You will conduct comprehensive assessments of deep wounds, skin tears, ostomies, severe burns, pressure injuries, diabetic foot wounds, and more.
Key Responsibilities:
- Collaboration and Assessment: Collaborate with the home health interdisciplinary team to review potential wound care patients for the Home Care Program and assess their eligibility for admission.
- Comprehensive Assessments: Conduct comprehensive assessments of deep wounds, skin tears, ostomies, severe burns, pressure injuries, diabetic foot wounds, and more. Determine treatment needs and long-term care plans while coordinating with healthcare providers.
- Wound Care Planning: Develop wound/ostomy care plans that incorporate input from the patient, caregivers, physicians, and peers. Collaborate with the interdisciplinary healthcare team to create a personalized care plan considering physical, financial, and emotional resources.
- Patient Education: Provide education and counseling to patients and families on wound care, pressure injury risks, infection prevention, and treatment measures. Educate and direct personnel in delivering patient care.
- Documentation and Communication: Document clear and concise progress notes reflecting the implementation and response to the care plan. Coordinate communication and documentation between patients, the Home Care Program, medical professionals, and referral sources.
- Quality Improvement: Participate in departmental conferences, quality meetings, and performance improvement initiatives related to wound, ostomy, and continence issues. Assist in data collection for performance improvement.
- Compliance and Regulatory Affairs: Ensure compliance with state and federal home health regulations and organizational policies. Participate in surveys conducted by regulatory bodies as needed.
- Case Management: Review case loads and chart audits, developing corrective interventions where necessary. Ensure compliance with billing guidelines and regulatory standards.
Requirements:
- Education: Bachelor's Degree in Nursing required.
- Certifications: Current New York State Registered Nurse License and Certification as a Wound Care Nurse or Wound Ostomy Care Nurse.
