Senior Living Healthcare Program Overview

Optum is seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our Senior Community Care team in Salem, OR.

Job Summary:

We are looking for a dedicated healthcare professional to provide high-quality care to our patients in assisted living and other community-based settings. As a member of our interdisciplinary team, you will work closely with patients, families, and caregivers to develop and implement personalized care plans that address the physical, mental, and social needs of our patients.

Responsibilities:
  • Conduct comprehensive assessments of patients' medical and functional status
  • Develop and implement individualized care plans that address the unique needs of each patient
  • Provide education and support to patients and their caregivers on disease management, medication adherence, and self-care
  • Collaborate with the interdisciplinary care team to ensure seamless transitions of care and coordination of services
Requirements:

To be successful in this role, you will need:

  • A graduate degree in Nursing (MSN) or a related field and current licensure as a Nurse Practitioner
  • Experience working in senior living or geriatric medicine
  • Strong communication and interpersonal skills
  • The ability to work independently and collaboratively as part of a team
