Lead Case Manager

Job Overview:
- This is an exciting opportunity to join our team as a Bilingual Lead Case Manager at Saludos Hispanos Family Connection Center in San Francisco.
About the Role:
The successful candidate will provide comprehensive case management, family advocacy and information and referrals to individuals and families. They will conduct assessments that identify strengths and needs, and implement these findings into individual service plans. This role requires strong interpersonal communication skills, knowledge of early childhood education and intervention, and familiarity with Microsoft Office and working knowledge of databases.
Responsibilities:
- Provide case management services to individuals and families, including conducting assessments and developing individualized service plans.
- Collaborate with other service providers to ensure comprehensive coordination of care for participants.
- Conduct outreach efforts, including presenting at community events and updating outreach materials.
- Manage referrals and coordinate distribution of referrals to case managers.
- Support and facilitate weekly support groups, including organizing guest speakers.
- Provide training and coaching to case managers on best practices.
- Responsible for translation of agency rules, announcements, and communication between families and staff.
- Other duties as assigned by the Family Support Director.
Requirements:
- Bachelor's degree preferred; or additional relevant professional experience with Associate's Degree in Social Work, Human Services, Family Services, Counseling, or related field.
- Bilingual Cantonese/English or Spanish/English required.
- Minimum 3 years of experience working with children and family services, including case management.
- Excellent interpersonal communication skills.
- Familiarity with Microsoft Office and working knowledge of databases.
