We are seeking a skilled Licensed Vocational Nurse Rehab to join our healthcare team at Las Palmas Medical Center. As a key member of our rehabilitation team, you will provide high-quality patient care and work collaboratively with our multidisciplinary team to achieve optimal patient outcomes.
Key Responsibilities- Provide direct patient care under the supervision of a registered nurse, including assessing patient needs, developing and implementing individualized care plans, and evaluating patient progress.
- Collaborate with the healthcare team to ensure seamless communication and coordination of patient care.
- Participate in patient education and counseling to promote health and wellness.
- Contribute to the development and implementation of rehabilitation programs and services.
- Maintain accurate and up-to-date patient records and reports.
- Current LVN licensure that allows practice in the state of Texas or Compact state.
- Basic Cardiac Life Support certification.
- Associate's degree in Nursing or equivalent.
- Rehabilitation nursing experience preferred.
- Effective communication and interpersonal skills.
- Ability to work collaboratively as part of a team.
- Strong problem-solving and critical thinking skills.
- Rehabilitation nursing experience.
- Computer skills and proficiency in electronic medical records.
- Bilingual in English/Spanish preferred.
