About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at DaVita Inc. as a Nephrology Care Specialist. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and contributing to the success of our organization.

Responsibilities
  • Provide direct patient care to individuals with end-stage renal disease or chronic kidney disease, including administering medications, monitoring vital signs, and performing various medical procedures.
  • Collaborate with healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and implementing strategies to address them.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and optimal patient outcomes.
  • Participate in quality improvement initiatives to enhance patient satisfaction, reduce readmissions, and improve overall healthcare outcomes.
Requirements
  • Current LPN-LVN license in the state of practice required.
  • Current CPR certification required.
  • IV certification required if required by state regulations.
  • High school diploma or equivalent required.
  • Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system.
  • BONENT or CHT certification preferred or as required by state regulation.
  • Minimum of one year's experience in a hospital or clinical setting preferred.
  • Basic computer skills and proficiency in MS Word and Outlook required; functional proficiency in computer systems of DaVita within 90 days of hire required.
