Clinical Nurse Case Manager

About the RoleThis position is a core member of our patient-centered medical home teams led by physicians/providers. You'll work collaboratively with multidisciplinary teams to develop and implement personalized care plans that address complex medical and psychosocial issues.

Responsibilities- Provide care management and coordination services for at-risk patients
- Develop and implement personalized care plans in collaboration with multidisciplinary teams
- Conduct thorough assessments and identify high-risk patients requiring intervention
- Collaborate with providers, community resources, and family members to ensure seamless care transitions
- Graduation from an accredited school of nursing
- Oregon Registered Nurse License upon hire
- 3 years of clinical experience as an RN
- Bachelor's Degree in Nursing (preferred)
- Experience in home health, skilled nursing facility, and/or community health nursing (preferred)
- Managed care experience and/or utilization management experience (preferred)
- Care/Case Management experience (preferred)
