Job Summary

We are seeking a skilled Medical Social Worker to join our team on a per diem basis. The ideal candidate will have experience in biopsychosocial assessment, crisis intervention, and short-term counseling. This role will involve working with patients and their families in the ER setting, providing advocacy, linkage to resources, and planning for transitions of care.

Responsibilities
  • Conduct biopsychosocial assessments and develop plans for patients and their families
  • Provide crisis intervention and short-term counseling
  • Link patients and families to community resources and support services
  • Collaborate with the interdisciplinary team to develop and implement care plans
  • Provide education to staff and other hospital departments on psychosocial aspects of care
Requirements
  • Master's degree in Social Work or related field
  • MSW-Medical Social Worker certification
  • ACSW or LCSW licensure
  • Strong clinical skills in biopsychosocial assessment and clinical interventions
  • Excellent collaboration and communication skills
  • Knowledge of transitions of care and community resources
Working Conditions

This is a per diem position, working variable shifts, including weekends. The ideal candidate will be able to work independently and as part of a team, with excellent time management and organizational skills.
