Job Description

Job Summary

This Occupational Therapist position will provide therapy to patients in their homes based on patient need and medical direction. The ideal candidate will have excellent communication skills and be able to work effectively with patients, caregivers, and healthcare teams.

Responsibilities

  • Assess patient needs and develop personalized treatment plans to restore and return maximal function
  • Provide 1:1 care with patients, focusing on adaptive equipment and patient/caregiver education
  • Participate in ongoing process improvement, patient satisfaction initiatives, and program development activities
Required Skills and Qualifications

To succeed in this role, you will need:

  • Bachelor's Degree or equivalent from an accredited occupational therapy program
  • Master's Degree preferred
  • 1 year or more of relevant experience in home care/VNA settings preferred
  • OCCUPATIONAL THERAPIST, REGISTERED (OTR) certification
  • Basic Life Support (BLS) certification within 90 days of hire
  • Valid driver's license
