Job Overview:

Mobile Healthcare Partners, Inc. is seeking a skilled Chronic Care Management Nurse to join our team.

Job Description:

This role will involve providing telephonic care management services for patients throughout the health system under the direction of a patient's provider.

  • Cooperate with patients and their families to develop personalized care plans
  • Work collaboratively with healthcare teams to provide outreach and engagement services
  • Provide education and guidance to patients on chronic disease management and self-care
  • Identify gaps or barriers in treatment plans and coordinate referrals as needed
  • Educate patients on healthy lifestyle choices and encourage participation in disease management programs
Qualifications:

To be successful in this role, you will need:

  • An unrestricted and current license to practice as a LVN/LPN/RN in New York State
  • A minimum of 3 years of experience in a care management setting or case coordination role
  • Strong knowledge of chronic disease states, including chronic kidney disease, diabetes mellitus, and congestive heart failure
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
Estimated Salary Range:

$65,000 - $85,000 per year, depending on experience and qualifications
