Colorado Visiting Nurse Association, affiliated with Care Synergy, is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in hospice and palliative settings.

Job Summary:

The LPN will work collaboratively with the interdisciplinary team to deliver comprehensive care to patients and their families, focusing on symptom management, education, and support.

Responsibilities:
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Collaborate with RN Case Manager, Charge RN, or assigned RN in the coordination of care to ensure Plan of Care (POC) is being followed.
  • Communicate patient and family information, status, and level of care to RN, manager, provider, and other IDT members.
  • Assist with coordination and successful completion of patient transfers.
  • Educate patients, families, caregivers, and community regarding hospice philosophy, palliative, and home health treatments.
Requirements:
  • Minimum Education: Graduate of an accredited school of practical nursing.
  • Minimum Experience: Twelve months nursing experience as an LPN and six months nursing experience in the last three years.
  • Must have an active Colorado LPN license or a license from the state of primary residence, if participating in the NLC (Nurse Licensure Compact).
  • Valid Colorado state driver's license and proof of auto insurance.
  • Current CPR certification.
