Responsibilities
  • Evaluate patients and create personalized care plans to ensure their unique needs are met.
  • Communicate effectively with patients, families, and the care team to ensure seamless care coordination.
  • Provide emotional support and comfort to patients and their loved ones during difficult times.
  • Collaborate with the care team to develop and implement effective care plans that address the physical, emotional, and spiritual needs of patients.
Qualifications
  • Graduate of an accredited school of nursing with a current state license as a registered nurse.
  • At least 2 years of nursing experience in a clinical care setting, with a preference for hospice experience.
  • Reliable transportation and ability to effectively lift at least 25 pounds.
