We are seeking a skilled Physical Therapist to join our team at Silver Lake Healthcare Center. As a Physical Therapist, you will play a vital role in helping our residents achieve their rehabilitation goals.

Responsibilities:
  • Evaluate and develop treatment plans for patients/residents based on their medical charts and evaluations.
  • Implement treatment plans through direct treatment and education of the plan of care to team members.
  • Attend interdisciplinary meetings to relay clinical findings and make recommendations to the rehab team.
Qualifications:
  • Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
  • Valid or pending State Physical Therapy License.
