Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Novant Health. As a Clinical Pharmacist, you will play a critical role in delivering optimal medication management across all dimensions of care, every time.
Key Responsibilities- Facilitate pharmaceutical care services, medication utilization systems within assigned services and care areas to ensure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulatory standards.
- Complete critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug-related problems.
- Partner as an active member of a multidisciplinary team and collaborate with healthcare providers within decentralized patient care areas to provide patient-centered care.
- Makes appropriate evidence-based, patient-centered medication recommendations.
- Participate in the management of medical emergencies.
- Provide discharge medication review, reconciliation, and counseling as appropriate.
- Maintain proficiency in and actively engage in central dispensing functions and operational programs.
- Oversee pharmacy operations and technician practice/activities.
- Maintain proficiency in preceptor roles and actively emulate education/mentorship skills.

Qualifications- Education: 4 Year/Bachelor's Degree in Pharmacy, required. Doctorate of Pharmacy, preferred.
- Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, required.
- Licensure/Certification: Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required. Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role; excludes PRN team members who may not be eligible to take certification exam, required.
