In this role, you will be responsible for delivering exceptional patient care, working closely with clients and their families to develop personalized care plans that meet their unique needs and goals.

• Provide one-on-one skilled nursing care to clients in their homes
• Conduct thorough assessments of clients' physical and emotional needs
• Develop and implement individualized care plans to promote optimal health and well-being
• Administer medications, perform wound care, and provide other necessary interventions
• Collaborate with healthcare teams to ensure seamless care transitions

To be considered for this rewarding role, you must possess:

• A current NJ nursing license in good standing
• One year of clinical experience strongly preferred
